What is checked during the field commission for issuing the Act of Permanent Care for the LCC with a conclusion

To verify the fact of permanent external care for a person who requires permanent care, according to the conclusion of the LLC, an on-site commission from the local administration (district or city council) is conducted to issue or refuse to issue an Act of the fact of care on a professional basis, that is, with material (monetary) compensation from the state or to establish the Fact of Care without compensation.

To do this, the commission must use the tables below and assign appropriate scores, taking into account the physical ability of the person being cared for to move and self-care, as well as checking the appropriate condition and the care provided by the caregiver himself, who carries it out on an ongoing basis. The scale table provides that the selected positions must reflect the real actions of the person in need of social services over the previous 24-48 hours (if necessary, the assessment is carried out over a longer period).

The basic activities scale assesses the daily activities of a person in need of social services using 10 criteria related to self-care and mobility (see table below). The basic activities are assessed by the sum of the points assigned to each person in need of social services for each section of the scale. The maximum score is 100 points, the minimum score is 0 points.

The scale for assessing the ability to perform complex actions evaluates the complex actions of a person in need of social services using nine criteria. The level of performance of complex actions is assessed by the sum of the points determined for each person in need of social services for each section of this scale.

The maximum score is 27 points, the minimum is 10 points.

The purpose of filling out the card is to assess a person’s living skills by determining quantitative indicators of the need for social care services on a non-professional basis, which is based on unified indicators of the level of mastery of skills in performing elementary and complex actions
in 14 categories of life activities.

The results of the assessment are stated in the conclusion and applied according to the principle:

the lowest level — from 0 to 111 points;

basic level — from 112 to 142 points;

satisfactory level — from 143 to 244 pointsand;

good level – from 245 to 314 points;

high level — from 315 to 345 points.

The assessment of living skills starts from the lowest level. The next level of assessment is reached if there are affirmative answers to all five characteristics, for which five points are respectively credited. If at the lowest level, the answers to four or fewer questions are affirmative, the level of mastery of the skills is assessed as the lowest. If at the levels from basic to high, the answers to one or two questions are affirmative, the level of mastery of the skills is assessed as the previous one; if there are three or four affirmative answers, this level of mastery of the skills is credited.

A person in need of social services may have a high level of living skills in one category, but basic or the lowest in another.
